Payment Information

We know insurance details can sometimes be confusing.  If you are in a Point of Service Plan or HMO, prior to your appointment please contact your insurance carrier or primary care physician to determine if a referral is needed.  Remember to bring all applicable insurance cards, forms and be prepared to pay your co-pay at the time of your visit.

Patients with injuries due to accidents involving third party payers will be fully responsible for medical charges and payment at the time of service. Examples of third party liability accident include but not limited to auto accident, department store accidents, ect.
