Medical records are copied upon receipt of proper authorization and reproduction fees are based on the guidelines of the Texas State Board of Medical Examiners. All requests must be submitted in writing.
X-ray films brought to our offices that are not copies will be returned to the original facility where they were taken. If you need to check out your x-ray films taken at our offices, please submit a request in writing and signed by the patient. Film requests require a 24 hour notice for pick up.
Disability forms are completed for a fee, which must be paid at the time the form is submitted. The patient section of all forms must be completed and the forms must be signed by the patient at the time of submission.
